Traditional methods of tracking resident care workflows, scheduling assessments, and documenting health data were often time-consuming, fragmented, and prone to error.
DSS developed Caribou Community Living Care (CLC) Suite to address the growing need for streamlined, efficient, and accurate resident care management within Community Living Centers. This solution automates resident care workflow processes for CLCs, and through VistA/CPRS-integration, it streamlines Minimum Data Set (MDS) care plan scheduling and tracking within an externally hosted environment.
The CLC Suite enhances clinical decision-making through real-time access to vital health data, such as active medications, allergies, and lab results, while also improving the scheduling and tracking of MDS care plans. MDS care plan scheduling aids in the creation and management of CMS regulated MDS Assessments. It also assists users in the accuracy and timeliness of data entry using real time CMS validation rules.
