DSS, Inc. Awarded Contract for Patient Case Manager (PCM) Solution Implementation at a Mid-Atlantic VA Medical Facility

Reinforcing how DSS is helping the Department of Veterans Affairs (VA) to become a High Reliability Organization (HRO), the company announced today that a Mid-Atlantic VA Medical Facility is implementing its Iconic Data Patient Case Manager (PCM) solution.

Through this new contract, DSS will offer a suite of applications within PCM that will assist the facility in ensuring the delivery of highly reliable, well-coordinated care for Veterans.

The solution will utilize real-time data by integrating with CPRS/VISTA to create a more optimized patient flow and patient safety-oriented solution and care plan.

Ultimately PCM will improve care coordination, help the facility to manage its SAIL metrics and enhance efforts around patient access, population health management, care coordination, community care, and public safety at care transitions.

PCM offers a cost-effective, physician-centric mobile solution for inpatient census management and efficient and structured shift-changes/sign-outs.
