Crescenz VA Medical Center (CMCVAMC) Deploys the DSS Iconic Data Patient Case Manager (PCM) Solution

The U.S. Department of Veterans Affairs (VA) is currently focused on transforming the VA into a high reliability organization (HRO), and is committed to empowering Veterans with more health care options under the MISSION Act. The CMCVAMC is one of 18 medical facilities in the country selected to participate in the overall high reliability initiative. Here are the five pillars of the VA’s HRO effort.
In alignment with this, the PCM VA HRO Platform supports the CMCVAMC HRO journey and the key priorities that Under Secretary for Health Dr. Shereef Elnahal outlined in his Sept. 8, 2022 town hall. The platform’s capabilities enable VA Medical Centers to improve care for Veterans and to achieve excellence on some of the most challenging and high priority performance measures including, for example, those for suicide prevention.
The PCM VA HRO Platform achieves this by injecting real-time data into key clinical and administrative workflows and by providing digital view management boards enabling proactive action. PCM will assist the CMCVAMC with:
Providing the best inpatient and outpatient care for Veterans
Achieving its HRO goals and the Undersecretary’s priorities
Enhancing its Strategic Analytics for Improvement and Learning or SAIL measure performance
Integrated into the facility’s live hospital information system, the PCM VA HRO Platform modernizes the legacy Electronic Health Record system addressing key gaps and workflow inefficiencies. The software also improves the situational awareness of care teams, enables better sensitivity to operations through data visualization, and has a track record of saving valuable VA employee time. This is essential to reducing employee burn out, improving employee experience, and recruiting and retaining a talented and motivated workforce.

In multi-centered prospective clinical studies, this approach has been shown to reduce communication failures and preventable medical errors.
PCM has been implemented at a number of VA facilities, and additional Veterans Integrated Service Network procurements are expected in government fiscal years 2022 and 2023.
