HB Mobile

HB Mobile is a Home Based Mobile Solution that allows HBPC Clinicians to perform a critical service of treating VA patients in their homes. This program helps Veterans maintain a better quality of life while receiving much needed care in their home, instead of an institution.

HBMobile is automated and electronic:

• HBMobile uses the template designs from DSS and provides a seamless automated system that drastically reduces
  the input time for the clinician and improves patient safety and accuracy.
  
• Patient information is automatically transferred from VistA to the caregivers’ secure tablet PC through a simple
  synchronization step.
  
• Note templates, assessments, and supporting documentation are electronically archived on the tablet PC for efficient, organized use as needed.
  
• Subsequent transfer of completed care information from the tablet PC back into the patients’ electronic record
  in CPRS is handled through the same synchronization process – reducing transfer time to seconds or minutes, instead of hours or days.
